Requested information for your MC3 consultation

Our goal is to provide consultations efficiently and effectively. When you call to initiate an MC3 consultation, a local Behavioral Health Consultant will ask you a series of questions and gather necessary information to ensure we provide a consultation that fully addresses your concerns.

Please have the following information ready when you make your MC3 consultation request:

  • Patient’s initials
  • Patient’s date of birth
  • Patient’s insurance
  • The best way for you to be contacted (e.g. office backline, cell phone)
  • Additional questions that will be asked by the Behavioral Health Consultant include, but are not limited to, the following: psychiatric, developmental and social history; relevant medical/surgical information; family psychiatric history; and risk concerns.

We understand that you may not have all of the background requested, however, the more information we have, the better resources and consultative guidance we can provide.
